          This codex is composed of three distinct sections. Part I: ff. 1-
     133; Part II: ff. 134-223; Part III: ff. 224-228. All three parts are
     written on sturdy brown paper measuring ca. 260 x 190 mm. with no
     identifiable watermarks.

          Part I: ff. i (parchment flyleaf from a Greek liturgical
     manuscript of the 12th century) + ii (paper) + 133, with a written
     space of 205 x 147 mm. Written in ca. 44 long lines; frame-ruled in
     hard point; prickings at corners of written space. I^^4, II-III^^8, IV^^4
     (-4), V-VI^^6, VII^^10, VIII^^8 (-1), IX^^8, X^^6, XII-XIII^^4, XIV^^6 (+1),
     X^^6, XI^^10 (-10), XII-XIII^^8, XIV^^4, XV-XVIII^^8. Quires signed by a
     later hand in Greek notation in lower margin, on recto. Written by two
     scribes. Scribe 1 copied ff. 1r-133v in a neat scholarly hand
     characterized by extensive abbreviations; Scribe 2 added notes on f. 4
     in a more cramped minuscule. Small initials and chapter headings, in
     red.
          Part II: ff. 90 (190 x 160) mm. Folios 134r-136r written in 2
     columns of 25 lines; folios 136v-223v in 26 long lines. I-IV^^8, V^^10,
     VI-VII^^8, VIII^^6 IX^^8, X^^10, XI^^8. Signatures in a later hand.
     Completed by a single scribe in minuscule, using pale brown ink. Some
     rubrication.
          Part III: ff. 5 + ii (paper) + i (contemporary parchment), with a
     written space of 243 x 165 mm. 52 long lines by a single scribe in tiny
     minuscule similar to that of Scribe 1 in Part I. I^^6 (-1). Signature
     added later.
          The codex is stained throughout; mending tape and worm holes
     frequently render the text illegible.
          Binding: s. xvi-xvii. Four chain-stitched supports link the quires
     and are laced into square-edged, flush, wooden boards with grooved
     edges. Colored, beaded endbands are sewn on fine cords attached to the
     boards. There are two twisted thread placemarks attached to the
     headband. The edges are bright yellow; the spine round and smooth with
     a spine lining extending across about one third of the outside of the
     wooden boards. Covered in dark brown goatskin, blind-tooled with an x
     surrounded with diamond-shaped tools stamped at random, within an outer
     border. One board is mended and both have small lumps in corners and
     center where bosses would normally be, underneath the present cover.
     There are traces of plaster where the leather is worn through over the
     lumps. There are two pins in the edge of the upper board, three
     corresponding holes going through both board and pastedown in the
     lower. Straps wanting. Rear flyleaf of the 11th century from a Latin
     liturgical manuscript.

          Written in Byzantium in the 14th century. Ownership note (15th
     century) of "Manolli [?] Notaras de sancta andrea" on f. 26r. Belonged
     to the Jesuit College of Clermont, Paris (no. 225; table of contents on
     f. iii verso and note on f. 1r: "Paraphe au desir de l'arrest du 5.
     juillet 1763. Mesnil."). Acquired by Gerard and Johann Meerman, ca.
     1773 (see Bibliotheca Meermanniana, v. 4, p. 22, no. 148, where it is
     described as "Mutilus, mucidus, squallidus et vermibus valde
     laceratus."); sale to Sir Thomas Phillipps (8 June - 3 July, 1824;
     stamp with no. 6758 on f. ii recto, tag on spine). Purchased from
     Laurence Witten with funds from the Jacob Ziskind Trust in 1957 (MS
     24).
